A program designed to follow patients for a minimum of 30 days providing expert guidance, education and support. Our team will coordinate community resources and help enhance communication between doctors, patients and their families. Our goal is to set patients up for success in navigating their healthcare needs and to avoid hospital readmissions for anyone newly discharged from the hospital.
Our Team will help assist with the following services and make sure there is cohesive communication between all parties.
